Social support – individual is assistance provided by a companion either in your home environment or to meet your social contacts so you can to participate in community life.
Services funded include:
• visiting services
• telephone and web-based monitoring services (including other technologies that help connect you with the community)
• accompanied activities (such as assisting the person through accompanied shopping, bill-paying, attendance at appointments and other related activities).
As a Social Support Individual provider we may use grant funding to purchase you IT equipment, including tablets, smart devices and internet subscriptions to help connect you with your family, carers and social groups.
This support is capped at $500 per client per year (or up to $1,000 in exceptional circumstances) in accordance with CHSP arrangements for other aids, equipment
and assistive technologies.

Our individual support sessions are Finnish CALD specific services, catering for Finnish linguistic or cultural needs.
We specialise in culturally appropriate services and encourage seniors to stay active. Our Social support individual program is delivered by volunteers to provide companionship and social support at home, and in the community.
Services are free of charge and depend on volunteer availability, language and other preferences.

This Multi-Service Centre also provides In Home support operating from Upper Coomera to the QLD/NSW boarder. In-Home services include; • Flexible Respite • Personal Care • Social Support • Domestic Assistance
