You can take part in our individual social support services for people living with dementia. We aim to help you be less isolated, supporting you to be more socially engaged by building connections and getting involved with your community.
Meaningful activities are organised around your interests, needs and abilities, supporting you to live as well as possible and where appropriate, connecting with your community. Activities can also be arranged for you and your carer together, to support and strengthen your relationship, giving you both an enjoyable experience.

In Darwin, we offer one-to-one social support for people with memory concerns or living with dementia this includes home visits and support to access appointments and social gatherings in the community.

You can build a package of services that best supports you in living the life you want. You can also choose the level of control you want over the aged care services and support you receive within your budget. A budget is the available funds to support your care. It can be a combination of government subsidy as well as your own and/or your family’s contribution. The CDC ethos aims to empower consumers to continue to manage their own lives by having control over the aged care services and support they receive.
