The Dementia Social Support service provides:
- Coordination of appointments and bookings; planning daily activities; liaison with family/carers as required.
- Monitoring medication, health and safety.
- Referral to other services as needed.
- Social support in home, centre-based and community outings.
- Assisting the individual to establish a support plan and network - identifying people they would like to assist them (i.e. family, friends, neighbours or our volunteers/paid staff ).
- Lending library of memory aid equipment and materials to assist clients to remain independent and at home.

Social support is for companionship and to help you access the local shopping centre and other social activities.

The City of Tea Tree Gully’s Social Support Services – Individual assists frail, older people to participate in community life through an ability to go shopping, pay bills and attend appointments.

Services include one-on-one shopping and medical transport.

Social Support can be provided in different ways at Nepean Food Services. Our 'hot run' volunteers visit people to provide a more direct contact with consumers. Nepean Food Services Meals On Wheels delivers a large range of nutritious meals to the homes of our consumers. We provide a variety of different types of meals such as Main Meals & Desserts, Mini Meals, Soups, Omelette's, Pasties, Pies, Sausage rolls, Modified Meals are also available on request and much more. It is the quality, nutrition and diversity of Nepean Food Services meals that provides our highest standard. Meals are delivered Mon-Fri between 11:30am- 1:30pm for deliveries that are heated and prepared.

Our staff and volunteers accompany people in the community to enjoy a meal and social interaction. Nepean Food Service's has continued a high COVID-19 Safety policy to maintain a safe environment for consumers, volunteers, families and staff who participate in programs that are offered.

We primarily provide phone support to people who need assistance to access the services they have been assessed and approved for. We provide training & support in groups and one to one for seniors on digital literacy through our Leep in Lab..

Uniting’s support workers can visit you at home for a cuppa, conversation, and companionship. They can help you to start a new hobby or project, play cards or games with you, and even support you to use digital technology so you can easily connect with family, friends, and your community. They can also accompany you to attend local events or classes, visit your favourite café or park, attend medical appointments,or give you an extra pair of hands to help with the shopping or bill paying.

If you’re finding it tricky to get out and about our team can take you to the shops and get you to and from your appointments and social outings. We can also help to organise your bills and appointments and help you set up and use your technology.
