Community Calls: Our Community Calls service (previously known as Silver Cord) is a Queensland wide telephone reassurance service aimed at improving social connections. We provide regular and pre-scheduled telephone reassurance services to suit your needs. All calls are made by friendly staff and volunteers, so you can look forward to a familiar and friendly voice on the other end of the line.
Home Visiting: Our home visiting services aim to improve social connections for those who may feel lonely, isolated, or would just like a chat or visit during the week. Whether you would like someone to just have a chat, have a coffee, or play a game, we are there with you.
Accompanied Activities: If you need help to get out and about and stay socially connected to your community then we can help you attend the shops, medical appointments and leisure activities.

Social support: working with you, assistance is provided one to one by a companion (paid worker or volunteer) either at home or for activities in the community. Staff and volunteers are trained in supporting older people and people living with dementia.
Activities are based on your preferred interests and needs and may include:
• home-based activities such as cooking, gardening, dog walking, reading, playing games, assisting with technologies that help your connect with friends, family and community
• accompanying you to do your shopping/bill paying, and attend appointments
• supporting you to connect/reconnect with community groups and activities such as social or sports club, participating in exercise, attending a book club, church, football or the movies, singing in a choir, visiting friends.
Where needed staff or volunteers can provide transport and accompany you during activities.

Social support assists a client to participate in community life.

Activities include:
• visiting services
• telephone based monitoring services
• assisting the person with shopping
and other related activities.

Social support is all about what makes you happy. Our job is to make sure you stay connected to your community.

Whether it’s going out for lunch, enjoying a game of bingo, bowls, or simply spending a quiet afternoon playing cards, we aim to match you with staff who share your interests, ensuring that you enjoy meaningful interactions that make you happy and keep you engaged.

This service aims to minimise clients social isolation. Service can include visits to clients home to chat and have a coffee, take clients out for a drive, assist clients to attend group activities to enable clients to keep in contact with the community.
