Our staff and volunteers deliver high quality individual social support to support frail older people through the delivery of individual social support, to maximise their independence in their home and in the community.
People eligible for this service are people with a disability (PWD) (65 years and over or 50 years and over for Aboriginal and Torres Strait Islander people).
We offer a range individual visits, accompanied shopping and outings to assist people enjoy each day, to make and maintain friendships, which also allows carers to take a break from their usual caring responsibilities.
Our Services may include:
• In-home social interaction
• Social support to events/functions
• Social support to appointments
• Respite for you or your carer
• Phone call/virtual check ins

Australian Unity Home Care Service - Bathurst
Australian Unity’s home services are suitable for older people who would like some extra support to get the most out of life.
We offer support with everyday activities like personal support, cleaning and gardening, as well as health care services delivered by qualified professionals, such as nurses, physiotherapists and podiatrists.

This program assists people who are frail aged, have a dementia or another disability and their carers to remain living independently at home. The Social Support program assists people develop social contacts and friendships to assist them to participate in community life and to access services and facilities so that social and geographical isolation is minimised and quality of life is increased. Participants are linked with a person on a one-to-one basis to provide services which best suit their needs. These services include - visiting service, accompanied activities to assist people shopping, bill paying, attending appointments and other related activities.
Usually service provided is one on one however may include more than one person where there is a couple requiring support. Assistance maybe provided in the customers home or in the community.
