St John's Community Care Innisfail
St John’s Social Support Individual provides assistance with your social needs and encourage seniors to stay active. We provide companionship at home, social groups, as well as transport and getting out and about. We provide flexible, responsive and person centred services valuing people with special needs, appreciating diversity. We will match the consumer and care staff and provide a culturally appropriate staff to support you with access in the community to participate in social activities. We work with you to create a Care plan that’s right for you, maintaining your wellness. Each individuals care plan & services are delivered, monitored, reviewed and reassessed regularly. St Johns staff undergo training for their role, ensuring quality care services are delivered. With over 25 years’ experience in aged care, we collaborate with individuals, families, health services, community groups and agencies in developing care plans/services to achieve person’s wishes.

St John Community Care (Companions) SA - South
An individualised service where a volunteer is matched one to one with a client, based on similar interests, skills and personality to provide companionship, social engagement and support to engage in activities that enable the older person to maintain a sense of meaning and purpose, even in times of increasing frailty. Activities may include social outings, home visits, getting out and about in the community, learning together, sharing interests (music, cooking,crafts, model trains etc), doing things together (eg. shopping /movies/ arts), capturing and/or sharing life stories and more. All activities are personalised and are designed around the needs and interests of the client.
Not Active Dementia Outreach Program
Outreach – Wollondilly Program
This program is specifically designed for people with Low to Medium level Dementia/ Alzheimer’s living in the Wollondilly area. The aim of the service is to enhance general health and wellbeing of people living with dementia in a stimulating and friendly environment
The program also provides respite for carers.
Groups operate from the Bargo area on Thursdays.
Our activities reflect the needs, interests and backgrounds of the individuals who attend each program or group.
The program involves a variety of activities, including:
• Arts and crafts
• Games and quizzes
• Memory puzzles and reminiscing
• Singing and exercises
• Outings/ day trips i.e. local parks, picnics, etc.
Who is eligible for the service?
• People with a diagnosis of dementia or memory loss
• People living at home in the Wollondilly area
• Carers of people with dementia

Assessment and Assertive Case Management
Lithgow Centre Based Day Care
Doreen’s Place is a day centre for frail aged people with priority given to people with dementia including Alzheimer’s. The service operates on a Thursday and Friday in a family home-like environment which provides appropriate socialisation for a small number of people and respite for their carers. Transport to and from the service can be provided if required within the city limits.

The Department of Health and Ageing provides funding to complement the day program at Doreen’s Place, specifically targeting the carer of a person with an ACAT assessment of Dementia or Alzheimer’s.
Doreen’s Place overnight respite program enables 1-2 persons to stay overnight between the two day visits (Thursday night).
