When you decide that you need some support at home, Uniting Communities’ home support team is here to help you to maintain your independence, health and wellbeing.
We offer a range of support, and our supportive and reliable team will help you to determine the best type of support which can include social activities, for example we run community social groups to give you the chance to make new friends, try new things and go on excursions, and can arrange for a volunteer to regularly visit you and offer friendship and companionship. If you have specific needs we will put in place support tailored for you – for example, wherever possible, we will try to match you with a carer who can support your cultural and language needs.
Vietnamese Centre Based Day Care (Social Support Group)
Based on the person-centred approach, CCCi staff together with service users and their family members/carers develop a care plan to respond to individual needs and design relevant activities to achieve individual client’s goals.
Care workers or trained volunteers, who speak clients’ language, provide assistance and support through a range of services and activities including
Telephoning and Web– based monitoring services
Visiting on a regular basis at your home or go for a walk or for a coffee and have a chat. On a home visit, a care worker can play a range of activities with clients and help with small tasks.
Accompanied activities – assist you through accompanied shopping, bill paying, accompany to such as medical, social, cultural appointments.
Fee: $5 flat rate per visiting and/or accompanied activities for full pensioners, or $10 for part-pensioners
If transport service is requested to take clients out of home, a small transport fee will be applied

Social support: working with you, assistance is provided one to one by a companion (paid worker or volunteer) either at home or for activities in the community. Staff and volunteers are trained in supporting older people and people living with dementia.
Activities are based on your preferred interests and needs and may include:
• home-based activities such as cooking, gardening, dog walking, reading, playing games, assisting with technologies that help your connect with friends, family and community
• accompanying you to do your shopping/bill paying, and attend appointments
• supporting you to connect/reconnect with community groups and activities such as social or sports club, participating in exercise, attending a book club, church, football or the movies, singing in a choir, visiting friends.
Where needed staff or volunteers can provide transport and accompany you during activities.
