Australian Unity’s home services are suitable for older people who would like some extra support to get the most out of life.
We offer support with everyday activities like personal support, cleaning and gardening, as well as health care services delivered by qualified professionals, such as nurses, physiotherapists and podiatrists.
Our friendly and dependable team will take the time to get to know you and tailor a support program to suit your individual needs. You’ll have a choice of team members to assist you and they will work with you as part of your expanded support team.
You’ll only pay for the services you use and agree to.

Being socially active is essential to maintaining good mental health and wellbeing. We can help you stay connected with your family and friends, so you can continue to do the things you love.
Our care team provides elderly social support that can help you indulge in your favourite hobby, visit family and friends, and even try something new. We offer a range of services so you can stay involved in your community and maintain your independence.
Activities can include visiting a café for a coffee or go shopping, attending local community events like football games, theatre, or other spiritual activities with like minded people, and helping you spend time with your friends at home.
Uniting AgeWell Social Support (UVT)
Volunteer-based individual social support matching a volunteer with a client to provide social connection either within the home environment or within the community. The programs goal is to support the person’s continued independence by meeting their need for social connection and support their engagement and participation in their community. Uniting AgeWell provides both volunteer visiting, as well as phone based social support through the Telelink (group-based) and Chat-a-Ring (individual) programs. Western District Health Service Community Support Services (CHSP)
Social support individual is assistance provided by a Community Support Worker to you while you are accessing the community, which is primarily directed towards meeting your need for social contact and/or company in order to participate in community life. WDHS does not begin delivery of this service until Monday 3rd October, so any new referrals will not be attended to until this date.
