UnitingSA - CHSP
UnitingSA offers Commonwealth Home Support Program services that assists older people and their carers to maintain their independence and support them to live at home and remain in their communities.
We provide one on one social support, social groups and transport services. We have a specialty with CALD backgrounds and have bilingual staff that speak a number of other languages, we welcome the opportunity to assist you.

The WA Country Health Service (WACHS) is a State Government organisation providing comprehensive health services to country West Australians.
WACHS spans 2.55 million square kilometres and is the largest country health system in Australia covering the whole of the State outside the metropolitan area.
Despite vast distances, WACHS provides health services to residents and visitors that include emergency and hospital services, population, public and primary health care, mental health, drug and alcohol services, Aboriginal health, child and community health and residential and community aged care services.
WACHS provides aged and community care services for older people across our communities, including aged care assessments, community care, residential aged care, subacute and acute care, and transitioning older people from hospital back into the community.
WACHS is a provider of CHSP services such as domestic assistance, personal care, meals and food services, and transport.

JCS Home & Community Support Services (CHSP)
A range of Social Support services are available at JCS, and may include assistance with
• shopping, banking and paying of bills,
• accompany individual to medical and social appointments, family and social visits in order to develop and maintain meaningful friendships,
• promoting social inclusion to build each individuals confidence, and maintain strength and capacity as much as possible.
All services provided by JCS are reflective of the diverse needs of individuals and specifically have a focus on an “Active Choices Active People” model of service.
Hutt St Centre - Homelessness Services
Individual social support is provided to improve or maintain a person's participation in community life and reduce the impact of social isolation. Social support provided to individuals can include: • Visiting • Telephone and web-based support • Supporting people to access and use technology that will enhance their community participation and links • Assisting the person through accompanied shopping and bill-paying, by attending appointments • Assistance to establish and maintain community links Although this type of social support is usually provided to individuals, it can be provided to more than one person eg if the support is being provided to an older couple.
