Hunters Hill Ryde Community Services
Aims to link a friendly visitor who will be in regular contact (usually up to 4hrs per month) with an isolated, house bound elderly person or a person with a disability. We offer a specific service linking Italian speaking visitors with the elderly Italian community in the LGA.
The visit will provide opportunities for the sharing of interests and support.
Our organsiation carefully matches volunteers to clients making sure that they suit your needs, share your interests and are willing to learn about you.

Social Support Individual Program is an entry level service, which is to assist older people improve their social connections, companionship and participate in community life. This is to help maintain their independence and enhance the quality of life while living at home.
The wellness, and reablement approach aims to help care recipients build confidence, strength, regain functional goals or targets to help the older person live independently and safely at home.
The activities include; one to one friendly home visits, accompanying a care recipient to appointments, help with shopping and daily tasks, establish friendships, connecting group interaction and outings, ongoing phone support, morning tea, lunches and transport if required.

Our social support individual services include in-home social visits, one-to-one outings and transport services. Attend any social function or outing eg. Drawing/ Hobbies/knitting set up; Painting, Music and or Dance (set up/assist); Reminiscence Therapy; Play games; Singing; Musical instrument –support eg. Set up; Television or online viewing support; Technology support; Outside walks; Reading

Our Social Support Team offer welfare checks (via telephone and home visits) to individuals who are socially or geographically isolated, frail or living worth a disability. We offer digital literacy support and help navigating My Aged Care and assistance accessing aged care services.
