Our CHSP Social Support Individual program helps provide companionship, either at home or in the community. It involves regular visits to help decrease social isolation, create new friendships, and increase independence.
We offer assistance to;
• Medical appointments via the provision of a staff member and transport.
• Help you complete your shopping.
• Help you attend community events.
• A staff member to help you access the general community for those everyday tasks, such as paying bills etc.
• Our services are free of charge
Our programs are flexible and responsive to your individual need. Our program promotes an environment that encourages and supports the maintenance of daily living skills and opportunities for social interaction.
