Our CHSP Social Support Individual program helps provide companionship, either at home or in the community. It involves regular visits to help decrease social isolation, create new friendships, and increase independence.
We offer assistance to;
• Medical appointments via the provision of a staff member and transport.
• Help you complete your shopping.
• Help you attend community events.
• Visiting you at home, or going for a walk, or having a coffee and a chat. During a home visit, staff members can help you with small tasks, such as checking fridge contents and use-by dates of food items. Helping you complete forms.
• A staff member to help you access the general community for those everyday tasks, such as paying bills etc.
• Telephoning and/or Web-based monitoring services (for example, making a regular phone call for a friendly chat, or making sure you are fine, or reminding you of scheduled appointments)
• Our services are free of charge
