Caulfield Community Health Service (CCHS) consists of centre, community and home-based health professional services and health promotion activities. We assist people in our community to improve and maintain their health.
Access and Support staff work with older people (aged 65 and over, and 50 and over for Aboriginal people), and their carers who are from different backgrounds and communities who are having difficulty finding out about services or applying for the services they need, like accessing Commonwealth Home Support Program (CHSP). Their role is to support people who are having trouble navigating the system and find services that suit their needs.
