Description

The Healthy Lifestyle and Wellness Program is a CHSP Social Support Group service type. The objective is to assist frail older people to participate in community life and feel socially included through structured, group-based activities that develop, maintain or support independent living and social interaction whilst facilitating their wellness and reablement goals.
The program will offer activities such as hydrotherapy, Stepping On, modified exercises, Tai Chi, yoga and yoga in a chair, music and singing groups, excursions, and group online activities, as well as information workshops and other group based activities that will increase health awareness and promote falls prevention strategies.
