Community Nursing Service provide and coordinate the delivery of accountable and responsible client centred care using an evidence based framework. Clinical services provided include;
• Acute and chronic wound management,
• Management of PICC lines and other venous access devices,
• Management of catheter and stoma care,
• Ongoing assessment, planning and evaluation of client care including relevant and timely referrals to other services,
• Supporting Hospital with early discharge and avoidable hospitalisation.
Other clinical activities provided include;
• The provision of end of life care and support to palliative care clients and their families/carers including pain management and basic nursing support,
• Hospital in the Home,
• The provision of chronic disease management activities such as, care coordination through the provision of self-management strategies, discharge planning from the inpatient services and ongoing case review
