CASE MANAGEMENT- AccessCare Southern (ACS) offers case management through a number of government funded care packages and programs for people over 65 years of age ( over 50mfor ATSI) in the Southern Region of Melbourne including:
• Short and Long term help and referrals to access a range of supports depending on your care needs, such as shopping assistance, personal care, cleaning services, transport, allied health, social connections, gardening, nursing, respite and home maintenance
• Linkages packages until 2018 ( we are accepting new clients until 31 March 2017)
• Community Connections Program for people who are homeless or at risk of homelessness
We also offer a Private Case management service
• Our qualified health professional staff with extensive experience in aged and disability care will help you identify which services best meet your needs. They will provide information and options. They will involve your family and carers, arrange and coordinate services.
