Based on individual assessment, qualified and registered allied health professionals work with the person to establish a tailored treatment plan that may include individual or group therapy, exercises, self-management of chronic conditions. Services are provided by or under the supervision of allied health professionals, with a preventative, maintenance, or restorative focus, to support the client to be independent. • Physiotherapy
• Formal counselling
• Speech pathology
• Nutritional advice
• Education in use of equipment that supports independence
• Exercise/gym classes/falls prevention
• Tai chi, Chi Ball
• Condition-specific (Parkinson’s, stroke/pulmonary conditions, arthritis, diabetes) self-management.
