Merri Health provides specialised support services that assist older people to live independently and safely in their homes and in the community. Our highly skilled Wellness at Home Team works alongside people and their family. We work in a manner that promotes wellness, re-ablement and independence supporting people by:
• Coordinating services
• Liaising with other medical providers
• Advocating on people's behalf
We accept referrals for people who would benefit from long term or short term case management and care coordination. Our services respect the cultural and linguistic diversity of our clients and interpreters can be arranged as required.
