Dementia Australia will no longer be accepting referrals for this service through the My Aged Care portal. However, this program will continue under different funding arrangements, with changes to program eligibility, including:
• Accessibility across all of Australia
• No restrictions for people who have other funding packages in place
• Availability to a wider range of ages
• No client contributions

Alternatively, your client can access our individualised dementia support services, by contacting the National Dementia Helpline on 1800 100 500.
