The Aged and Community Care Program is a primary community-based resource which provides an interdisciplinary service to support frail older people, younger people with disabilities and their carers. The program aims to keep the client independent within their community, proactively promoting health and independence, and self-management whilst assisting with timely interventions to prevent deterioration of the condition, and admission to hospital and or residential care.
