Grampians Community Health is a community health centre which provides leadership in the development and delivery of primary health care and community services throughout the Grampians region.

Active Respite Options is part of the Inclusive Communities Team of Grampians Community Health (GCH) which consists of dedicated people who coordinate a combination of government funded services to support people in their community. The Commonwealth Home Support Program (CHSP) supports frail older people living in the community to maximise their independence. This exciting program can be offered in a variety of different ways, such as:
• Social interaction and community connections
• Day trip outings
• Retreats
Our community engagement approach provides holistic service delivery across Stawell & Ararat, where clients are valued and actively participate in the activities offered.
