District Nurses conduct home visits for monitoring health, assistance with administering medication and support. Care includes assessment and monitoring, wound care, catheter care, palliative care, medication management, referrals to other services and care co-ordination. The District Nursing Service provides Home Nursing for clients needing follow-up care after discharge from hospital. Hospital in the Home (HITH) and Post-Acute Care (PAC) Programs are available via service plan agreements with discharging hospitals. District Nursing can also service DVA and TAC clients and can be contracted through Aged Care Packages to provide home nursing. Hours: 8:00am to 4:30pm weekdays and 08:00am to 12:00pm on weekends. The service also provides some clinics at the Community Health Centre for: Simple Foot Care; Wound Care; Stoma and Continence Care.
