Our nursing team provides a range of community based services aimed at supporting you at home. Nurses assess, refer, educate and support you in managing health conditions that impact on your ability to access the community and manage independently at home

• Pressure Injury prevention
• Continence Management and Aid Application
• Home Oxygen
• Medication management
• Provision of Information, community referral and future care planning
• Carer support and education
