HammondCare STRC Programmes are designed to support people who are 65 years or older and are living at home in the Inner West Area. The STRC Programme is an early intervention programme that offers both preventative and targeted care, aimed at delaying functional decline or targeted at specific functional goals such as safe mobility and falls prevention.
Programmes are delivered by a multi-disciplinary team (Allied Health, Medical and Care staff) with experience in designing and delivering water and land based restorative care programs for older adults. Programmes are delivered over a period of (up to) 8 weeks with the focus of each program varying depending on the individual clients’ goals and care needs. Services may be delivered in either, or both, a home care or a residential setting.
HammondCare’s Centre for Positive Ageing actively promotes restorative care programs for older people in the local community and to service providers.
