Community Connections Program (CCP) is funded by the CHSP Program to undertake work with adults within the local government areas of Hume and Moreland.
The program aims to provide assistance to people who are experiencing homelessness or are at risk of homelessness or who live in low quality accommodation and have multiple unmet health needs. The program uses an assertive outreach approach to locate and engage with potential clients. The program provides short term case management to assist clients to access mainstream and /or specialist health services and aims to improve social connectedness for the person depending on their needs.
