Description
Access goods, equipment or assistive technology to support the independence of older people living in the community.
The ILC GEAT service provides basic and essential items to help with daily activities such as personal care, communication, and mobility. A specialised assessment and recommendation may be required to be completed by an appropriate health professional to support referrals for some items.
How to Refer:
To help us best manage our referrals, please add as much detail as you can to the referral comments - referrer name and contact number, item/s required, and whether the client is also linked in with an allied health service. Where a health professional has already completed an ILC GEAT Application Form for the requested item/s, please attach and upload to My Aged Care.
