Description

MannaCare administers all types of Home Care Packages:

• Level 1 entry level service needs
• Level 2 to support people with low level care needs
• Level 3 to support people with intermediate care needs
• Level 4 to support people with high care needs.

Package care assists people (“consumers”) to remain living at home for as long as possible and enables them to have choice and flexibility in the way that the package is managed. All packages are offered under the Consumer Directed Care model of service.
A service plan is developed to meet each consumers goals and needs.
The types of services a package can offer include personal support, nutritional services, continence management, social activities and clinical care.
