Embracing our strong belief that "You Matter", our Care Management Team will work with you to tailor your care and supports through a combination of CatholicCare's extensive range of service offerings, as well any additional providers that you may choose to engage. If you choose to have your HCP fully managed by CatholicCare, you will enjoy a dedicated Case Manager who will work for you to maximise the value that you receive. Your dedicated Case Manager will monitor the delivery of your care and regularly speak to you about your current care needs. This care management approach means that your Case Manager can promptly adjust any of your supports if your needs change.
