Northern Sydney Home Nursing Service provides intermittent Community Nursing care to people in their home, and in clinics where available. The service requires that all patients have medical supervision from either a Medical Practitioner or a hospital, and the patient will be asked to provide consent for the service to contact the GP and other relevant service providers when necessary. NSHNS is not an emergency service. For urgent advice, patients are advised to contact their General Practitioner, or emergency services. The Service operates 7 days a week including an Evening Service 8.00am – 8.30pm The following services are provided with an aim to achieve independence and self-care: • Comprehensive Assessment • Oncology Support • Medication Education and Management • Wound Management • Palliative/End of Life Care • Short term Personal Care • Chronic and Complex Care, including Care Coordination and health coaching • Continence Care • Aged and Dementia Care
