The Social Support - Individual Service aims to assist frail older people to participate in community life and feel socially included through meeting their need for social contact and company.
Services include:
Home Visiting and
Accompanied Activities such as shopping, bill paying, attendance at appointments, going out for a coffee or to visit a place of interest.
Clients are matched up with worker who will visit or accompany the clients.
Clients contribute $7.50 per hour for a maximum of 2 hours support per week. This is direct debited once a month.
