To access government-funded aged care services you need an assessment to find out if you are eligible.

After your assessment you will receive an assessment result and, if eligible for services, a support plan. Your support plan will explain what services you are eligible for.

When you receive your support plan and what you do next, depends on the type of assessment you have and the services you’ve been found eligible for.

I received my support plan at my assessment

If you received your support plan during your assessment, usually this means you were assessed under the Commonwealth Home Support Programme. Your assessor will tell you what services you are eligible to receive or explain why you are not eligible.

I have not received my support plan and assessment result

If you did not receive your support plan during your assessment, then you were likely assessed for a Home Care Package, a short-term care option, and/or an aged care home.

After your assessment, the assessment organisation will review your information to determine if you are eligible for services.

You can discuss with your assessor how you would like to receive your assessment results. This can be by post, by email or your My Aged Care Online Account within 2 to 6 weeks after your assessment.

I have received my support plan and assessment result, what’s next?

When you receive your support plan and assessment result you will also receive the information you need to access these services.

If you aren’t eligible for government-funded services, your support plan may suggest other care options.

For detailed next steps, select the program below that is listed in your support plan:

  • Short-term restorative care
  • Residential respite care
  • Transition care
  • Aged care homes also known as Residential Care

You can also view your aged care information by logging into your My Aged Care Online Account where you can see your:

  • support plan
  • assessment result letters (if relevant to your situation)
  • aged care service referral details.

Note: If you told your assessor that you want your support plan available in your My Health Record, you and your doctors will be able to see it there. You can change your mind at any time by calling My Aged Care on 1800 200 422 and requesting for it to be shared or removed.

Fee-paying aged care navigation and placement services

There are many free Government supports available to help you find and connect with government-funded aged care services. However, some people decide to use fee-paying services for different reasons. If you decide to use a fee-paying service, we’ve put together some tips and recommendations to keep in mind. Learn more about these services.

You can check the progress of your My Aged Care application or assessment by logging into your My Aged Care Online Account using myGov. You  can also call My Aged Care on 1800 200 422.

Will the services I want be available?

Whether – and when – you can access a service will depend on the availability of service providers in your area. In the case of Home Care Packages, it also depends on the national wait time for your package. You can call My Aged Care on 1800 200 422 for advice on Home Care Package wait times, and for help with finding available providers.
