My Aged Care support plans now available in My Health Record

Your My Aged Care support plan can now be shared to your My Health Record within your myGov account. 

From 9 December 2024, you can share new and updated support plans with your healthcare providers through My Health Record. 

How do I share my support plan?

When you have an aged care assessment, your assessor will ask if you authorise sharing your support plan with your My Health Record.

Your support plan records what you discussed during your assessment and sets out the care and services that will help you. 

Who can access My Health Record?

My Health Record is a safe and secure place to keep your health information.

Having your support plan available in My Health Record allows health professionals, such as your doctor, nurses and allied health professionals, to see your support plan. This will give them a better understanding of your aged care needs and help them give you better care.

You and your healthcare providers can access your My Health Record at any time, including in an emergency.

Managing access to your My Health Record

You can manage access to your My Health Record from your myGov account to update who can see your support plan. 

You can choose to start or stop sharing your support plan within My Health Record at any time by calling My Aged Care on 1800 200 422.

Please note: This change applies to new and updated support plans. Support plans created before 9 December 2024 cannot be made available in My Health Record.
