How can I access this service?

You can access this service if you have been approved for permanent residential care or short-term residential respite services, or through the Transition Care Program.

Short-term care and support to help you regain your functional independence and confidence after a hospital stay.

Aged care homes

Suitable for older people who can no longer live independently at home and need ongoing help with everyday tasks or health care. Also called residential aged care.
