Restorative care management is a service that comes as a part of receiving short-term care through the Restorative Care Pathway under the Support at Home program. A care coordinator, through your provider, ensures that you receive appropriate care for your preferences, culture, and needs. They help with care planning and monitoring services. They will also help you to understand and access the restorative care and services you receive.
This service includes:
- home support restorative care management.
This service does not include:
- the administrative costs funded through prices on services, such as account management, billing, and claiming.
