Care management is a service that comes as a part of receiving ongoing care through the Support at Home program. A care partner, through your provider, ensures that you receive appropriate care for your preferences, culture, and needs, including end-of-life care. They help with care planning, coordinating, reviewing, and monitoring of your services. They will also help you to understand and access the care and services you receive.
This service includes:
- home support care management.
This service does not include:
- the administrative costs funded through prices on services, such as account management, billing, and claiming.

How can I access this service?
This service will be provided to all older people accessing ongoing services through the Support at Home program. Your aged care assessor determines your eligibility to access this program.
