Georgios enjoyed get-togethers at his local pool complex. He’d see his friends from the Greek community, and they’d swim, laugh and share tall tales.
But one day, Georgios slipped by the pool and fell hard on his hip. The accident limited his movement and stopped him from attending these regular catch ups.
Struggling to complete his usual daily activities, Georgios contacted My Aged Care. He had an assessment and was approved to access services under the Restorative Care Pathway through the Support at Home program. His assessor recommended an intensive 16-weeks of allied health care to get him back to doing daily tasks independently.
Georgios approached his local Greek-based Support at Home provider and met their restorative care partner Nikolas.
Together, they looked at Georgios’ Notice of Decision and support plan. They made a goal plan that included his specific needs, goals, preferences, services, duration, and budget. This plan included a physiotherapy, occupational therapy, podiatry, and domestic assistance. Nikolas explained how Georgios’ care was going to be monitored and evaluated, as well as what would happen when he finished the plan.
Georgios began attending physiotherapy appointments which looked at how he could strengthen his balance. He also went to group strengthening classes as his fitness improved. He talked about his progress with Nikolas during his regular visits and phone calls.
In the first few weeks, Georgios and Nikolas began discussing the exit plan. As the episode progressed, they agreed that the goals set at the start of the program had been achieved.
Georgios explained that he had lost some confidence following his fall and wanted to continue building his strength and balance. Nikolas submitted a Support Plan Review requesting a reassessment for Georgios. The reassessment gave him an approval to receive ongoing Support at Home services.
After a time, Georgios was confident to return to his friend’s catch ups. While he took greater care getting around the pool, the laughs and tall tales never stopped.
