我們的聯繫中心工作人員會與您探討您的需求與個人情況。若我們認為My Aged Care服務可以提供幫助,則會轉介您參加面對面評估。

我們的全體工作人員均經過培訓,懂得與不同情況的民眾溝通,也能隨時解答您的問題,幫助您找到最適合的方案。

撥打電話時,我需要準備什麼?

請備好您的Medicare卡,這樣會更方便我們討論您的需求。通話至少需要10分鐘,請確保您預留足夠時間。

電話裡會談些什麼?

我們的首要任務,就是讓您獲取自己所需的幫助。為此,我們會詢問:

  • 您的健康狀況
  • 您在家如何照顧自己
  • 您當前獲取的支持。

您的回答有助於我們瞭解您的需求,您也可能被轉介參加面對面評估。

我們會保存本次討論記錄,這樣在評估過程中您就無需重複提供相同資料。

誰可以支援我?

通話期間,您的家人、朋友或照護人員都可以在旁陪同。取得您的同意後,他們也可代表您發言。

若您希望長期安排某人代表您發言和行事,您可委託他們充當您與My Aged Care往來的代表。這樣,My Aged Care就可以與他們討論您的老年照護方案。

代言服務

您還可以通過Older Persons Advocacy Network (OPAN),獲取代言人幫助。取得您同意後,代言人即可代表您與My Aged Care交涉。

通話後要做些什麼?

掌握您所提供的資料後,我們會告知您是否符合參加面對面評估的條件。面對面評估可以準確判定您的需求。

若您需求迫切,我們也可在實施評估前,直接將您轉介給相關服務機構。

通信服務

若您需要幫助或想要通過英語以外的語言來溝通,可向Translating and Interpreting Service (TIS)尋求幫助。
