When Sarah had mostly recovered from a fall that had her in a wheelchair for over a month, she knew it was time to get help. A particular concern was the pressure sores she had developed from sitting. So, she organised an aged care assessment through My Aged Care.
After being assessed, Sarah was eventually allocated funding for ongoing Support at Home services. She found a provider and scheduled an appointment with Lucy, one of their care partners.
Lucy checked Sarah’s Notice of Decision and support plan, which said she could receive nursing, house cleaning, gardening services and transport services. They discussed which services Sarah would like to receive, how often and her preferences regarding workers.
Sarah really liked what this provider offered, so she told Lucy she would like to use their services as soon as possible.
So, Lucy explained the next steps. This included that the provider would draft a service agreement outlining terms, conditions, rights, and responsibilities. Sarah and Lucy then created a care plan together and agreed how they would use her quarterly budget.
They agreed to having a nurse assess Sarah’s skin issues, including her pressure sores, and setting up regular check-ins for ongoing care management. Lucy sent a copy of the service agreement and care plan soon after Sarah signed the agreement.
Once back at the office, Lucy organised Sarah’s services.
After a fortnight, Lucy visited Sarah to see how everything was going. She checked what services had been delivered. Sarah’s assessment had a plan for managing her skin integrity and outlined regular nursing services. Sarah was very happy with her care, and they organised Lucy’s future visits.
After regular nursing care, Sarah’s skin integrity improved. Lucy continued to work with Sarah and coordinate her daily care activities. This included arranging Sarah’s preferred gardener and ensuring Sarah had taxi vouchers to attend her GP appointments.
