This privacy notice describes how the Department of Health, Disability and Ageing (department) collects your personal information through an assessment organisation. The department contracts the assessment organisation to undertake aged care needs assessments.
We collect your personal information when you provide information to an assessor at an assessment. We also collect your personal information from third parties, such as family members or health professionals, who the assessor speaks with about your needs.
The collection of this information is authorised by the Aged Care Act 2024.
