Located within HammondCare Caulfield on the Caulfield Hospital campus, The Glens is medium, two-level home care home that supports people with low to high frail aged care needs in single and shared rooms. The home is surrounded by beautiful, landscaped gardens and neighbours HammondCare’s Caulfield Village, which offers cottage-based care for people with dementia. HammondCare is committed to seeing each person as an individual and caring for their total wellbeing. Care team members receive specialised training and use a relationship-based approach to provide 24/7, registered nurse-led care. They learn about each person's history, passions, values, and hobbies to help them maintain a fulfilling life. Additional personalised services such as pastoral care and volunteer services are provided to enhance comfort and quality of life.
