When to get reassessed

If you’ve had an assessment in the past, there may come a time when you need to be reassessed.

The assessment you had would have been based on the needs that you had at that time. However, circumstances can change. 

You may find that since your last assessment, your needs have changed. Perhaps some other daily tasks have become more difficult for you. 

Choose the situation that fits you best, and take the appropriate steps:

You already receive subsidised aged care services

If you are receiving subsidised care but find that your needs have changed:

  1. Speak to your current provider to review your care plan 
    They will review your care plan and see if there’s something more they can do to support you.
    You can find your current care plan by logging in to your My Aged Care Online Account. 

  2. Your service provider will contact your assessor
    Your assessor may amend your support plan to meet your changing needs or they may determine that a reassessment is required.

You had an assessment in the past, and weren’t eligible

Even if you weren’t found to be eligible in the past, your needs may have changed since your last assessment. You can ask for a reassessment at any time.

When should I consider an aged care home rather than help at home?

You might consider moving to an aged care home if you can no longer live independently at home. Or perhaps you need more help than your current carer, family or friends can provide.

Will I need to change providers if I move from Commonwealth Home Support Programme services to a Home Care Package?

Possibly. Some service providers offer both. If you’re currently receiving Commonwealth Home Support Programme services, you should talk with your provider. You can find your provider details online, through your My Aged Care Online Account.
