Registration of a Supporter Form (PDF)

This form can be used if you want to request to register a supporter to help you find and access aged care services.

If you want to access Australian Government-subsidised aged care services, you need to register with My Aged Care.

Using this form, you can request to register a supporter to help you make and communicate your aged care decisions. This includes requesting and accessing your information in My Aged Care. Registered supporters must comply with certain duties under the new Aged Care Act, including that they must declare, and avoid or manage, conflicts of interest.

Your prospective supporter will also need to agree to the registration so it’s important you speak to them about your wishes before you start this process. This form can also be used by your active, appointed decision maker (if you have one) to request to register as your supporter. Your active, appointed decision maker does not need your consent to register as your supporter.

Registered supporters must comply with certain duties under the new Aged Care Act, including that they must avoid or manage conflicts of interest.

If you need to declare a conflict of interest or have already been registered as a supporter and a conflict of interest later arises, you have a duty under the new Act to declare it.
