Our nursing services provide comprehensive care across the Geelong region, supporting clients to maintain independence and well-being at home and in the community.
Community Nursingoffers a seven-day-a-week, 24-hour home nursing service, including after-hours support. Staffed by Registered and Enrolled Nurses,this service delivers care to people eligible for CHSP along with a number of other funding streams. Teams are based at Belmont, Corio, and Newcomb Community Health Centres (CHC), providing nursing care after hospital discharge, palliative care, wound care, independence with diabetes management, and continence care
Community Health Nursing includes Diabetes Education, Smoking and Vaping Cessation, Key Contact Person and Refugee Health. Operating Mon–Fri from Anglesea, Belmont, Corio, Newcomb, and Torquay CHC sites, with focus on preventative health care and self-management of chronic diseases
