The Wodonga District Nursing Service provides home based nursing care to Wodonga and surrounding areas in Victoria. We provide high quality patient-centred care including acute and chronic wound Care, palliative care/symptom management, medication management, monitoring of vital signs, diabetic management, compression therapy/lymphoedema management and catheter care. Continuity of nursing care is provided in the transition of a patient from the hospital to the home environment.We assess and provide strategies and referral processes which assist the patient to remain in their home environment. This service charges a minimal fee for care to assist in keeping the service sustainable in accordance with the CHSP Client Contribution Framework. Information on the fees and fees policy can be provided on entry to the service.
