The Wodonga District Nursing Service provides home based nursing care to Wodonga and surrounding areas in Victoria. We provide high quality patient-centred care including acute and chronic wound Care, palliative care/symptom management, medication management, monitoring of vital signs, diabetic management, compression therapy/lymphoedema management and catheter care. Continuity of nursing care is provided in the transition of a patient from the hospital to the home environment.We assess and provide strategies and referral processes which assist the patient to remain in their home environment. This service charges a minimal fee for care to assist in keeping the service sustainable in accordance with the CHSP Client Contribution Framework.
