Frankston City Council - Community Care
This provider offers services through the Commonwealth Home Support Program.
Council's Group Social Support is an outings and excursion based program with 12-seater mini-buses. Client's are collected from their home supported by a volunteer and staff member. Client's are able to select from a regular brochure with a variety of outings destinations to elect outings that are of interest to them. Walking Aids can be accommodated but client's are required to be adequately independent to self-manage activities of daily living including personal hygiene and bathroom needs.
