Central Gippsland Health Service - Home Support Services
This provider offers services through the Commonwealth Home Support Program.
Our District Nurses conduct home visits to a broad range of people who require support, which can include assistance with medication, hygiene, health monitoring and wound care.
Services are provided 7 days per week between 8.00am to 8.00pm. Our Continence Nurse provides expert care, education, and support to individuals experiencing bladder control issues. They develop personalised management plans to improve continence, enhance quality of life, and promote dignity. Our Diabetes Educators specialise in supporting people living with diabetes. They provide personalised education, practical advice, and ongoing support to help individuals manage their condition effectively.
