We specialise in delivering in-home supports to people in living the community who have experienced deep, persistent disadvantage, social exclusion, and people who are experiencing or are at risk of homelessness. Our focus is on providing skilled and professional Social Workers with the time to get involved where family or social networks may not be available, to give a true holistic approach in partnership with our clients. Referrals to our organisatio are varied, but mostly our support includes: - referrals into other services for long term support (such as My Aged Care) - case management to support clients to link into care they have already been approved for - advocacy - assisting people to maintain connections with healthcare and attend their appointments - assistance with housing applications - assisting people to reconnect with their community - supporting people with matters relating to Services Australia (eg applying for pensions)
