The Dementia Nurse Practitioner provides support to both clients and family members living with Dementia across Gippsland. They work in conjunction with the GP, and can provide both a primary and secondary assessment of the client with cognitive and or memory impairment, to assist with early intervention and diagnosis within the scope of the Gippsland Dementia Plan.
