Community Nursing Service provide and coordinate the delivery of accountable and responsible client centred care using an evidence based framework. Services include • Acute and chronic wound management; • Management of PICC lines and other venous access devices; • Management of catheter and stoma care; • Ongoing assessment, planning and evaluation of client care including making relevant and timely referrals to other services; • Supporting Hospital with early discharge and avoidable hospitalisation. Other clinical activities such as • The provision of end of life care and support to palliative care clients and their families/carers including pain management and basic nursing support; • The provision of chronic disease management activities such as, care coordination through the provision of self-management strategies, discharge planning from the inpatient services and ongoing case review.
