JCS Home & Community Support Services
This provider offers services through the Commonwealth Home Support Program.
JCS offers a flexible transport program for private and self funded individuals who are seeking assistance with transportation to attend medical appointments, visit family in nursing homes, hospitals and to attend social functions and activities that are important to the individual.
Our Transport program aims to assist individuals to remain independent and promote social inclusion within the community.
With the assistance of a trained support worker, we offer a responsive and flexible transport service to suit the individual needs and requirements.
All services provided by JCS are reflective of the diverse needs of individuals and specifically have a focus on an “Active Choices Active People” model of service.
